New Delhi [India], August 27: Cosmetic surgery in India is increasingly being approached with an emphasis on natural-looking results, individual proportions and personalised treatment planning, according to Dr. Pinky Devi Ayyappan, plastic surgeon and founder of Pink Apple Aesthetics.
While cosmetic procedures have historically been associated with dramatic transformations, Dr. Ayyappan says consultations are increasingly centred on achieving subtle refinement while retaining the patient's individual appearance.
"Patients are increasingly asking whether the outcome will look natural and whether people will be able to tell that they have undergone surgery. The objective should be to enhance an individual's existing features rather than erase what makes them unique," said Dr. Ayyappan.
According to her, an "overdone" appearance is subjective and can sometimes result when a procedure is planned without adequately considering a patient's existing facial proportions, body structure, skin characteristics and overall appearance.
"A nose, for example, should not be planned independently of the rest of the face. Similarly, breast augmentation is not simply about choosing the largest possible implant, while liposuction is not about removing as much fat as possible. Good aesthetic planning involves proportion, anatomy and the individual's goals," she said.
Growing emphasis on informed decision-making
Dr. Ayyappan attributes the changing approach in part to greater access to information. Patients can now research procedures, recovery periods, potential risks and different treatment approaches before consulting a surgeon.
Social media has also influenced expectations around cosmetic procedures. While visual platforms have increased awareness of aesthetic treatments, filters, image editing and highly curated photographs can sometimes create unrealistic expectations.
"Surgeons have a responsibility to explain what is realistic, what is medically appropriate and what is likely to work with a patient's natural features rather than simply agreeing to every request," Dr. Ayyappan said.
She added that patients should consider the long-term outcome of a procedure rather than focusing only on the immediate postoperative appearance.
Natural does not mean no change
According to Dr. Ayyappan, a natural-looking result does not necessarily mean an insignificant change. A well-planned procedure can produce a noticeable improvement while maintaining harmony with the patient's existing features.
For instance, a patient considering rhinoplasty may seek a more refined nose without creating an artificially narrow appearance. Similarly, a patient considering breast augmentation may be looking for improved volume and proportion rather than a significant increase in size.
"The goal is not necessarily to make a procedure invisible. It is to make the result appropriate for the individual," she said.
Proportion remains central to aesthetic planning
Aesthetic procedures require consideration of the relationship between different facial or body features, rather than treating one area in isolation, Dr. Ayyappan said.
For facial procedures, factors may include the relationship between the nose, chin, cheeks, lips and overall facial structure. In body-contouring procedures, height, body frame, skin quality, fat distribution and existing proportions can influence treatment planning.
As a result, two patients requesting the same procedure may receive different recommendations based on their anatomy and individual objectives.
" There is no universal perfect nose, breast size, waistline or facial shape. Aesthetic treatment should be personalised rather than based on a fixed template," Dr. Ayyappan said.
Technology as a planning tool
Advances in photography, imaging and digital simulation have also provided surgeons with additional tools to communicate potential changes to patients.
However, Dr. Ayyappan cautioned that digital simulations should not be treated as guarantees of surgical outcomes. Individual tissue characteristics, healing and other biological factors can affect the final result.
Clinical assessment and detailed consultation therefore remain important components of treatment planning.
Consultation plays a key role
Before undergoing cosmetic surgery, patients should discuss their expectations, medical history, anatomy, treatment options, recovery period and potential complications with a qualified plastic surgeon.
Dr. Ayyappan said a consultation may sometimes result in a recommendation for a different procedure--or no procedure at all--depending on the patient's circumstances and expectations.
She also encouraged patients to ask about a surgeon's qualifications, experience with the proposed procedure, expected recovery and potential risks before making a decision.
Personalisation in the Indian context
India has significant diversity in facial features, skin characteristics, body proportions and aesthetic preferences. Dr. Ayyappan therefore believes that there cannot be a single template for an "Indian aesthetic."
"The patient should not be made to fit a trend. The treatment should be designed around the patient," she said.

Looking ahead, Dr. Ayyappan believes aesthetic medicine may continue to move toward greater personalisation, with patients seeking results that complement their appearance rather than necessarily creating an obviously operated look.
She advised individuals considering cosmetic surgery to begin by identifying what they want to improve and why, rather than choosing a procedure solely because it is trending or associated with a celebrity.
"The best cosmetic result is not necessarily the most noticeable one. Often, it is the result that makes a person feel more comfortable and confident while allowing their individuality to remain visible," Dr. Ayyappan said.
